If you’re not a member or if you’re just doing a quick search, use our public search tools to locate doctors, hospitals and other participating health care providers.

Always verify that the provider you choose is participating in our network before you receive services. While it is our goal to provide accurate information, our provider network is subject to change without notice. Providers in all areas of our service region are included in the provider search. Please be sure to select the correct product (PPO, etc.) when beginning any provider search.

*Please note that the transplant network is not the same as our standard network; providers reflected in the provider search tool may or may not also be contracted to perform transplant services. Please contact Customer Service at the phone number on the back of your ID card for assistance to set up contact with a case manager for these services.

Coventry Health Care partners with MHNet to deliver behavioral health benefits to our members. Most members must have these services pre-authorized before accessing them.

Coventry Health Care Freedom PPO and Optimum PPO Level 3 includes Coventry National Network providers. To find Level 3 providers, click this link and select Freedom / Optimum Level 3 under the product list. Level 1 and 2 providers can also be found by using this link.

Please note: Coventry National Network providers may not be available in all areas.

Some Coventry Health Care plans include access to EyeMed Vision providers. To find out if your benefit plan includes EyeMed, please check with your Benefits Administrator or call Customer Service. Some Coventry Health Care plans include access to Heartland Dental's network. To find out if your benefit plan includes Heartland Dental, please check with your Benefits Administrator or call Customer Service.

Fully Insured, Group PPO, PPO Plus and Platinum PPO Networks National Provider Search

Out-of-Area Providers: PPO Members

PPO members on an out-of-area plan and living outside the service area use the Coventry Health Care National Network. Dependents living outside the Coventry Health Care service area and enrolled in the Passport program may also access the Coventry National Network for in-network benefits.
