Small Business Group Plan

If you're a Small Business Group plan member (covered by an employer with 50 or fewer employees), your 2016 dental coverage is provided by Delta Dental of Washington (Delta Dental)* and uses the Delta Dental Premier® and Delta Dental PPO℠ dental provider networks.

Please note: If your employer has not yet renewed for 2016 and you have dental coverage, your dental coverage provider is Group Health Options, Inc. and the plan is administered by United Concordia. When your employer renews, your dental coverage may change. Check with your human resources representative for more information.

Your ID number
Your Delta Dental coverage does not have its own identification number. When you or a family member covered on your plan receives dental care, you will use the Group Health medical plan subscriber's member ID number, which appears on the subscriber's Group Health member ID card. If you're not sure who the subscriber is for your plan, call Group Health Customer Service toll-free at 1-888-901-4636.

Choosing a network dentist
A list of Delta Dental Premier® and Delta Dental PPO℠ dentists is available through Delta Dental’s Find a Dentist tool. Call your selected dentist's office to make your appointment and let them know you are covered by Delta Dental of Washington.

Choosing a non-network dentist
You are not limited to using a Delta Dental network dentist. You may use any licensed dentist. If you choose a non-participating dentist, you will be responsible to have the dentist complete your claim forms and to ensure that the claims are sent to Delta Dental. Claim payments will be based on actual charges or Delta Dental's maximum allowable fees for non-participating dentists, whichever is less. You are responsible for any balance remaining after the claim is paid.

What to bring to your appointment
Bring your Group Health member ID card and, if you or the person receiving care is a dependent on someone else's plan, bring the subscriber's Group Health member ID number. If you're not sure who the subscriber is for your plan, call Group Health Customer Service toll-free at 1-888-901-4636.

Claims and billing
Your network dentist will submit claims to Delta Dental of Washington on your behalf. If there are charges for services that are not covered, the dentist will bill you directly for those services. If you have questions about a claim, call Dental Customer Service at 1-800-554-1907, 8 a.m. to 5 p.m. Monday through Friday.

*If you have renewed onto the 2015 Core Bronze HSA or the 2015 Connect Bronze HSA and you or a family member are under age 19, your dental coverage is provided by Group Health Options, Inc. The plan is administered by United Concordia Dental (UCD) and uses the UCD Advantage Plus dental provider network. For more information about your dental plan call UCD Dental Customer Service at toll-free 1-866-568-5994, Monday through Friday, 5 a.m. to 5 p.m.
